Looks like an Anubias rhizome that is regrowing.  It’s leaves may stay small or may end up growing bigger as the plant returns to better health.  It also looks like there may be some bits of other plants stuck on it.  Maybe some moss, something from the Vesicularia group most likely.  And maybe a couple tiny leaves from something else entirely (the thin, light green bits stuck on the rhizome).  If those leaves are actually growing then the plant is very nutrient deficient.

I did a little picking at it and the stuff on the upper right that looks fern-like came away. I was left with the rhizome and the green roots. The roots weren't emanating from the rhizome but at the crown of it it. I trimmed and planted front and center in my tank; may get a better picture when I get some leaves that aren't torn up. The rhizome is only about an inch long. Honestly, it had the appearance of a carrot as far as the striations around the rhizome. I'll post if I get some new growth.
