Is this Ich? Or something else?

I have been treating my betta for fin rot. I was using kanamycin and salt (1 tbsp: 1 gallon) and then added Maracyn 2 because I read that minocycline can be synergistic with kanamycin for possible resistance. This fish has been treated twice in past 8 mos with kanamycin.  The temp is 81F.  Water parameters 0 amm/0 nitrite/ 10-15 nitrate

anyway, i noticed white spots on his side today.  See pic.  

Should i treat for ich? Unfortunately i leave on vacation tomorrow. So dont have anyone to remove the water and retreat when i am gone.

Its not really a clear picture so i can not really tell but your nitrates is a bit high so I recommend doing a water change before you go on your trip and if you just noticed it, it might be stress ich

I agree with @ryan1. Drop the nitrates to give your Betta the best chance of surviving while you are on vacation

Do a 50% water change, feed him and go. Fish can go a week without food easily, sure the might have seen better days, but they should survive. One exception is  fry though

Need to see more photos or a video for any sort of a clear ID on what's going on.

I see the spots you're talking about, I just need to see a bit more clearly and to see the fin rays themselves to check for ich as well.

What meds do you have on hand to treat this?
