What’s on my fish?

1 Congo and 2 rummies have it. White patches almost like wounds from rubbing against something. It’s mostly on the top. All fish are swimming normally. All are eating normally. This is a 75 gallon planted with heavy filtration. I have about 12 corrys, 10 rumnies, 6 Congo tetras, a couple ottos, a betta, ton of shrimp, and 2 dwarf frogs. They had ick a few months ago but I treated it and they’re cleared up. I do a 30% water change every week. Given a varied diet of frozen foods, flakes, dried tuboflex worms, & crushed snails.  
 

Temp - 76-78° they’re under a vent 

ph 6.6

gh 8-10

kh - 1-2

ammonia - 0

nitrites - 0

nitrates - 5-10

Could be columnaris that causes white patches on the back and head that's a gram negative bacterial infections that spreads more quickly at temperatures over 75 and can kill in 24hr to 7 days depending on the stain the most effective treatment is kanaplex and jungal fungus clear fizz tabs containing nitrofurazone following this treatment plan as you have shrimp and dwarf frogs I wouldn't treat with kanaplex or nitrofurazone they can be harmful to shrimp and possibly your frogs what I would do is two courses of maracyn2 active ingredient minocycline  back to back it won't harm your shrimp or frogs @lmhicks101

I thought I sent this last night but am I swapping the jungle and kan for the maracyn2 and repeating the treatment for 2 sets totaling 10 days?

What I would do is a full course of treatment do a 50% water then do a second course of maracyn2
