Suspicious Green Neon Tetra

After putting my new 13 green neon tetras in a 5-gallon quarantine tank, I put in the med trio. On the second day (today) I'm seeing one neon tetra separated from the group and swimming unnaturally. Its body is leaning to one side, and it is having trouble staying right sided. 

The parameters of the tank are 0 ppm ammonia, 0 ppm Nitrite, 7.6 ph, and <10 ppm nitrate.

If anyone knows what the problem is, please help me deal with this and thank you.

Update: Unfortunately, today the fish is no longer with us. Its swimming problem had quickly turned deadly to the point where it was spiraling uncontrollably. I had to put it out of its misery and euthanize it. 😢
