She's in a 6.5 hospital tank 

 

She has like an open sore, any recommendations for treatment

What I would do is large daily water changes and add a double dose of prime to get your nitrite down 5ppm of nitrite is extremely toxic to fish that could have left her susceptible to secondary infections I would treat with a high level of aquarium salt 1 table spoon for 1 gallon and maracyn2 in food feeding a small amount twice a day for 7 days that way you can do daily water to get your nitrite down while treating remember to only put back in what you take out so if you do a 1 gallon water change put 1 table spoon of salt back in as salt only removed though water changes guppies do best with a pH of 7-7.5 @Ron Uni

Oh goodness, 5 ppm nitrites, I miss read thinking the nitrates were the nitrites. Yes like @Colu, double or even triple dose prime and make it less harmful for a short time why you are trying to get the nitrite down. Do a big wc, then smaller ones too. Agree with @Colu with this food recipe.

I did do a water change though just to be cautious.

If your nitrite are zero then you have two options use the medicated food or dose the tank  maracyn2 or kanaplex would be my go to antibiotic treatment's in this case
