Ammonia - Nil
KH/Buffer - >80
Water Temperature - 75ish

This is Otto, Otto needs help :(

Has been treated with Rid-Ich Plus, API Pimafix and Melafix,
ParaCleanse will arrive in a day or two ... no idea what it is,
any suggestions?

Your nitrite is incredibly high. How long have you had detectable nitrite? Did you ever go from detectable ammonia and nitrite to 0 on both?

I'm not super knowledgeable on various diseases, but you should start with a 50% water change to lower the nitrite and dose extra prime if you have it, to help it not affect the fish as much.

Less then 10 ppm is high in a 60g planted tank? Fluctuates between o and 10, 10 on the high end.

I do 25% twice a week, just done yesterday

Any nitrite is dangerous for the fish. Though, I believe, not as dangerous as ammonia.

Your nitrate level is fine. 

Also, I read it as more than 10ppm, not less than 10ppm.

I would holed off on any further treatment and concentrate on getting your nitrite to zero at that level it's extremely toxic to your fish is probably the cause of what your seeing you can also add prime to help detoxify nitrite @Dave H
