How do you stop salt from doing this?

I store my salt in a air tight container out of the sun, but it keeps clumping up together like so, any tips on how to prevent this?

Add some dried pasta or any dried grain to help absorb the excess moisture.  Just make sure its easy to spot when you are using it

can I put the grains in a bag or must I spread it out?

as long as it is a porous bag or it won't work can't see the problem but it might breakup when damp. 

I would try big pasta so I can just lift it out.  I use corn in the table salt cellar again big and easy. 

If it has got very damp  might be worth drying in low oven before adding something

in the old days, they used to put rice in with salt in the shakers out in places like cabins that werent used all the time. , that or you could stick a dessicant package in the salt.
