White spots on face

I have this in-going disease with my geophagus sveni.   It is I’m pretty sure hole in the head disease.  I quarantined him with fritz aquatics paracleanse and the problem is coming back. I then purchased tetra lifeguard and treated my whole tank (60 gallon with some guppies bristkr nose plecos. Canister filter and sponge filter and Uv filter  and it seemed to be working but then this morning once again he’s getting these little with zits on his face only. Perhaps he wasn’t treated properly- but I followed the 5-6 protocol quarantine treatment.  Maybe it’s the sand substrate that is hosting parasites ? .  I do weekly or bi weekly water changes.  You can see the last photo - a new white dot / pimple looking thing Is present again. Makes me very sad. Any feedback is appreciated.

Hexamita doesn't usually form a cyst like that... I would suspect ich before HITH. It might be the result of stress, or potentially even dietary issues.
