There is alot of white flakes or particles floating in the water and on the plants not sure if fungus or what it is

Water is great water hardness is off the chart though but thats been the case been running the tank for 4 mo now its stocked

Not sure i started to notice white stuff floating around did a water change and now looks worst no new fish nothing new

Please help I dont k kw what going on in my tank just did a water change yesterday
